Introduction/Informed Consent Statement
RMC Research Corporation has been contracted by SEANet to establish an
online database on service-learning quality and sustainability.
The database will help local and state users determine the status of
programs at the local, state, and federal levels, and plan
professional development services. Changes will be charted over
time, for a period of up to three years. By logging in to this page,
you agree to complete your profile information and the survey. You
will be asked to provide a password, which you should retain for
use when you return to this page. You may stop answering the survey
at any time, and you may skip any questions you do not wish to answer.
Your name will not be associated with any computed results. Data
will be aggregated at the school, district, state, and national

Representing Your School or District
This database is designed for responses to be entered at the school
level. If more than one person from your school completes the
survey, every user's response will be aggregated together and
reported as an average for the school. School level personnel
can view results for their individual school. Data for schools
within the same district will also be aggregated and can be
viewed by district. Data from districts within the same state
will be aggregated and may be viewed by state-level Learn and
Serve coordinators.
